6. Safety Forms

Each team must complete and submit the following safety forms during the competition. For more detailed information about the iGEM Safety Program, click here.

Safety Check-In Form

Opens: TBD

Some organisms and parts present risks beyond what is ordinary for lab work in synthetic biology. As your project progresses, you should consider the risks presented by each organism and part you plan to use. The White List can help you with this.

Before you acquire or use any organism/part that is NOT on the White List, you must submit a Check-In. Check-Ins allow the iGEM Safety Committee to help you ensure that you will work safely with these riskier organisms/parts.

Safety Form (Preliminary)

Due Date: June 29, 2018

Part 1.1 is intended to collect contact information for you team throughout the iGEM season for any and all Safety questions. Please make sure you use an email that you will check daily.

Part 1.2 includes a short questionnaire form about the laboratory where you will work on your project, your laboratory space and your safety equipment.

Part 1.3 is a short questionnaire about your primary project idea. Its main purpose is to encourage you to think about the safety of your project as a whole, rather than focusing narrowly on individual parts. It can also help you think about future applications of your project, and what safety health environmental issues would be important to consider.

IMPORTANT: You must update this section if your primary project idea changes.

Safety Form (Final)

Due Date: September 7, 2018

Part 2 of the Safety Form includes New Parts you worked with and also allows your PI to review Part 1 of the Safety Form prior to submitting the completed form.

Part 2 must be signed by an Instructor or Primary Contact of your team, so please plan accordingly.
